Photo Booking FORM

Photo day is your dancers opportunity to put their concert costume back on and have their picture taken by MDA's photographer for a beautiful portrait photo.

 

All digital images taken will be made available for selection shortly after photo day. Digital images are $20.00ea or $15.00ea for 4 or more. A non-refundable booking fee is payable as follows: First item for first dancer $60.00 (includes 3 digital photos), then each additional item per dancer $30.00 (includes 2 additional digital photo).

Please arrive with hair and make up completed for your photo, as per concert day requirements.
